Every day there are a few lab results from previous ED visits that need to be checked and in some cases, acted upon. Examples are blood cultures, urine cultures and some various lab results.
It is the responsibility of the provider in the CDU to review these results in the "SJH ED Results" in basket each day and take action as necessary. The following is a guide on how to complete this task.
1) Open the SJH ED Results Inbasket
In Epic, open your general inbasket and navigate to the "Results" box.
2) Pharmacist Review
PharmDs will write comments via the "Follow-up" function. Comment scan be viewed by hovering over the message portion of the Result summary. These will be forwarded to CDU provider as well – The CDU Provider is responsible for:
3) "Done" results that require no further action
In the example above, because a negative blood culture requires no further action, this result can be marked as "Done" without doing anything else.
Sometimes a result will have already been reviewed by pharmacy. They are able to comment on the appropriateness of antibiotic treatment, but they will not "Done" a result. If you open a result and pharmacy has already reviewed the result and no further action is needed, please "Done" the result.
4) If a result needs further action, take it. And Document it.
Here is an example of a result that needs further action. A positive blood culture.
To deal with this result, please click the "..." section at the top and choose "Result Management"
In this case, the patient will need to be called to return to the ED. Go ahead and do this now (see note on how Dr. Saxon has already done this for this patient)
To document your intervention, check the box on the result you are dealing with, Uncheck the "Route" button on the result note, enter in a note describing your intervention, and check "Also file as a note" before accepting.
5) Only "Done" a result that has been definitively been managed. If a result cannot be "done"ed now but still needs further action, please "Mark as New"
Once you have accepted your result note go back to the Inbasket. If your intervention needs no further follow up (you changed an antibiotic to match a culture result) you can now "Done" the result. If the patient still needs further care despite your intervention, DO NOT "Done" the result at this time.
In the above example, the patient needs to return to the hospital. This result should only be "Done"ed once the patient has returned to the ED and the result has been definitevly been managed.
